Physicians have a unique opportunity to screen fall risk and to promote and preserve independence and health in all persons across the care continuum.
Reducing risk factors known to contribute to injurious fall events in the home, community, or healthcare facility starts with incorporating basic screening into all medical visits. The Hendrich II Fall Risk Model can be used in office practices and across populations to document and quantify fall risk and thus reduce risk factors whenever possible to prevent injurious fall events.
Screening for Fall Risk and Promoting Good Health
- A physician-specific, web-based learning module, delivers content on the importance of assessment in the areas of cognition, mobility and gait, medications, elimination patterns and their relationship to falls.
- Experts from geriatrics, pharmacy, nursing, and community health have organized the content into an efficient, CME-approved course available in 2014.
